We don’t have any specifics of what happened during the accident, but it was said that nobody else was involved. Amazon Prime Video and the producers of Gen V released an official statement following the passing of Perdomo:

Shooting for the second season of Gen V was said to have been planned for April, but after Perdomo’s passing, it looks like the shoot has been delayed indefinitely. We don’t know how they’re going to deal with Perdomo’s character moving forward, but his current and previous collaborators have all been posting up tributes to him online:

The last we saw of Perdomo’s character Andre, he was one of the heroes along with Marie (Jaz Sinclair), Jordan (Derek Luh/London Thor), and Emma (Lizze Broadway) who have been confined in some kind of power-neutralizing room.
We don’t know if it’s Vought or the government who has gotten to them, but we shouldn’t expect them to stay in confinement for too long. While the real heroes have been nerfed though, Vought has already started touting Cate (Maddie Phillips) and Sam (Asa Germann) as the new Guardians of Godolkin.
The arc of Andre could go several ways, but no doubt the loss of Perdomo is a huge blow to the series and the fandom.
No release date has been announced for the next season of Gen V.

The synopsis reads:
Thirty-five years after the shocking murders of three teens, an infamous killer returns on Halloween night to claim a fourth victim. When 17-year-old Jamie comes face-to-face with the masked maniac, she accidentally time-travels back to 1987. Forced to navigate the unfamiliar culture, Jamie teams up with her teenage mother to take down the psycho once and for all.
The film is directed by Nahnatchka Khan who had also helmed Netflix’s Always Be My Maybe; she was also the showrunner for Fresh Off the Boat which also starred Randall Park who is also in this movie. Other stars in the film include Olivia Holt, Julie Bowen, Charlie Gillespie, and Liana Liberato.
With Shipka being established as a horror genre icon thanks to Sabrina, it’s going to be interesting to see her be a pseudo-Marty McFly in a slasher movie. I’ll admit, I was kind of expecting her to lean into drama after Sabrina, but I guess there’s no harm in doing some more outrageous roles from time to time. Even Shipka’s TV dad, Jon Hamm takes comedic roles (and absolutely nails them).
Watch out for Totally Killer when it hits Amazon Prime Video this Halloween Season on Oct. 6.

Showrunner for Riverdale Roberto Aguirre-Sacasa also tells EW about the crossover:
“We’ve been talking about having Sabrina visit Riverdale since season 1, so it’s thrilling that this is finally happening, as part of our ‘Riverdale’ special event. It’s also perfect that she shows up to help Cheryl Blossom during her hour of greatest need. Everyone on set lost their minds — I think fans will, too. It’s really fun and special.”
Though Sabrina and Riverdale exist in very different worlds, the characters do crossover all the time in Archie comics, and it would be great to see how Sabrina fits into this world. Though she may have lost her show, maybe this means that we can see more of her character (as well as other characters from her show) pop up in the future—much like what happened with John Constantine and Legends of Tomorrow.

“Cheryl (Madelaine Petsch) is performing a dangerous spell, one that may mean the difference between life and death for a beloved member of the Blossom family, according to a logline description of the episode. Luckily, she’s getting a much-needed assist from everybody’s favorite 20-something witch, the iconic Sabrina Spellman (Shipka), visiting from Greendale in this years-in-the-making crossover.“
Sabrina visits Riverdale when her episode airs on Dec. 7. You can catch all four seasons of Chilling Adventures of Sabrina now on Netflix.
